If you wish to be a new client this is my Current Intake Process:
- Book an initial appointment using my acuity HIPAA compliant scheduling system with included intake forms. Please fully come the intake form. 2. For your appointment please print the paperwork and complete for your session. This is found under the tab “initial appointment paperwork.” Also any required additional paperwork should be brought with you to the session.
Things to know before completing the intake form. I do not provide: couples therapy, Substance Abuse Counseling, Domestic Violent relationship treatment, nor perpetrator treatment (As these are areas of weakness for me).
Areas that I do provide treatment: trauma, PTSD, Veterans, Mood Swings, Irritability, Bipolar, Psychosis, Body Image Issues, Self Esteem Issues, Self Harm, Toxic Relationship Patterns, Sexuality Issues, Eating Disorders, Insomnia, Behavioral Issues in Children, Academic Issues, Developmental Concerns with Children, Aggression in Children, Autism impacting the family and child (or adult) and more….
Last but not least: I require that in working with minors that both biological parents or legal guardians give consent for treatment, unless stated differently in a legal document (if the parents/ guardians are not together). I explain this more under the tab “working with children.” There are some situations in which I would not require that (ie. if the parent is incarcerated etc) but that would need to be discussed on an individual basis. This isn’t optional, you have to follow your custody agreement.
I’m only in network with a select group of insurance companies (Humana, United, BCBS, Tricare, Beacon, United Medicaid, LA Healthcare Medicaid) and thus if I do not accept your insurance it will be private pay. Also, if I am unable to verify your benefits for any reason or am not paid by your insurance company…you are still responsible for the cost of treatment.
I apologize in advance. I do try to make everything accessible and streamlined so that I can properly care for my active clients but still have availability for new clients. This process was developed due to an unfortunate misuse of my information being too accessible on social media. And thus I require intake forms completed or referrals from Providers prior to phone contact. Thank you for your patience and understanding & I will be in touch with you soon!
